This form must be submitted HOA each time you request approval for exterior building alterations or any type of construction to the exterior of your property - including, but not limited to, fencing, patios, decks, retaining walls, building additions and/or modifications, sheds, etc. Briefly describe the request for construction and/or alterations. Attach a plan that includes a drawing of the structure and details (color, style, etc.) regarding materials being used. Please be sure to include a copy of the original plot plan showing the construction or alteration. Completed forms can be returned either (1) via email by completing this form and hitting send or emailing the completed from to Info@ReeseManagement.com; or (2) via fax (215) 661-2800; or (3) mailing completed from and accompanying documents to Reese Management Company, PO Box 62226, King of Prussia, PA 19406.
You, as the homeowner, are responsible to obtain any permits necessary for the work being completed. A copy of the request, approved or denied, will be issued to you. Thank you!
THIS REQUEST IS GOOD FOR SIX (6) MONTHS. IF YOUR PROJECT IS DELAYED FURTHER THAN SIX MONTHS YOU WILL NEED TO RESUBMIT THE FORM.
